Echinococcosis is common in many parts of the world. However, bone and muscular echinococcosis are rare and often misdiagnosed. We report three cases: the first case was a 38-year-old man with a primary sacral hydatid cyst, the second case was a 37-year-old man, and the third case was both with primary muscular echinococcosis of the adductor muscles of the left thigh. Treatment was surgical in all three patients. Healing was obtained for 2 muscular sites, whereas recurrence was characteristic of the bone localization despite several surgical resections.
