Elevated liver enzymes are frequently observed but overlooked in typhus. Moreover, acute hepatitis associated with typhus has rarely been described in the literature. Here, a case of typhus presenting with acute hepatitis was described and the relevant literature was reviewed. Although sudden fever, headache, and rash (including eschar rash and maculopapular rash) are considered potential indicators of typhus, variable elevated liver transaminases, even acute hepatitis, may be another striking feature of typhus that clinicians should be aware of in an endemic area.
