Hymenolepis nana infestation is usually diagnosed in school-aged children and is very rarely reported in adult patients in Saudi Arabia. We encountered a Saudi Arabian adult patient with H. nana infection . The patient had a chronic history of vague abdominal pain and diarrhea. He had negative laboratory investigations and a negative stool examination report. After misdiagnosis of his symptoms as irritable bowel syndrome and ineffective treatment for irritable bowel syndrome, he was readmitted. After thorough but negative physical, systemic and laboratory examinations and microscopy of stool specimens, the patient underwent a thorough ileocolonoscopy. He was found to have H. nana infestation and his symptoms resolved after treatment with praziquantel. Our study supports the notion that colonoscopy with careful examination of the intestinal mucosa is a useful diagnostic approach for patients with parasitic infections and negative stool examination results.
