Cryptosporidium has become a major cause of diarrheal disease worldwide, especially in young children and patients with infectious or iatrogenic immunodeficiencies. The authors report a case of mild cryptosporidiosis with delayed diagnosis in a stem cell transplant recipient, probably due to confusion between digestive cryptosporidiosis and acute gastrointestinal graft-versus-host disease. Clinical and parasitological improvement was observed after a 3-day course of nitazoxanide and especially tapering of immunosuppressive therapy.
